Our Services| Case Management
The case managers at CAP work with clients to assist them in living healthy and empowered lives. Case managers make sure that clients are aware of their healthcare options and support them in accessing and maintaining involvement in healthcare and treatment. To support clients in healthcare, case manager conduct a comprehensive needs assessment. Together, the case manager and the client prioritizes need and develops goals and plans to meet those needs. Case managers also provide information and referrals for housing, financial assistance, mental health services, substance abuse treatment services, public benefits, insurance, food assistance, transportation, employment, and education. Case management assistance is also available in Spanish through the bilingual case managers.
The first step in working with a Case Manager is to become a CAP client.
